Dosing and Administration
Drug Calculation Program:
The CHEO Drug Calculation Programs are a medication safety tool designed to assist with the calculation of pediatric drug dosages and infusion rates. The tool was developed for use at CHEO and is based on the concentrations and dosages used at that institution. Patient specific dosages and infusions rates are calculated by entering a patient weight into the appropriate entry fields. | Layout. Lay out drug information on order forms, monitoring forms, and pharmacy labels in an identical manner. Pay attention to indentation, alignment, and spacing of information, as well as how many pieces of information are strung together on one line, in order to optimize readability. |
| Terminology. Use identical terminology on all forms and labels. For instance, if the generic name of the drug is used on the preprinted order form, then the same name should be used on monitoring forms and labels |
| Sequence. Ensure that the order of information on all forms and labels is consistent to facilitate cross checking. (For example, drug name always appears at the top, then concentration, dose, route, etc.) By doing so, the reader can form expectations about where to find information on a label and where to look on the order form to compare that information. |
